Is docivault built specifically for clinics?

No. It is built for the exchange between a provider and the people and organisations it sends records to. Clinics are one setting where that exchange happens; insurers and other providers are the same pattern.

Can a referral stay private until it is ready to send?

Yes, that is what the Private space is for. Upload it into Private, and move it to Client only when it is ready to send. Both are built and available now.

Is the document exchange itself built yet?

Yes. Workspaces, contacts, the three spaces, uploading a document, moving it between spaces, previewing common formats in the browser, comments and sharing by an expiring link are all built and available now. Document requests are in progress. Bulk import from Drive or OneDrive, and a complete record of every view and download, are not built.
